Division of Social Services: Case Manager

About the Role

• Assess individual and family needs, determining eligibility and identifying appropriate services.
• Provide access to necessary supportive services such as housing, health, government entitlement
and general social services information to new immigrants, low-income families and older adults.
• Ensure each community member is given the necessary services, case management and follow up to fill ongoing service needs.
• Make referrals to appropriate service providers in the community.
• Provide ongoing support to individuals to ensure they get the most out of the social services
available to them.
• Participate in training, special events and conduct workshops or presentations if needed.
• Any other duties assigned by supervisor and Executive Director.

  • Bachelor's degree in a directly related field or an equivalent combination of directly related experience, training, and/or education preferred.

  • General computer literacy.

  • Experience working with social programs to assist individuals as needed.

  • Culturally competent, self-motivated professional committed to social and economic equity.

  • Strong work ethic and demonstrated passion for working with diverse populations.

  • Must be a self-motivated problem solver with excellent analytical and problem-solving skills.

  • Skill in oral and written communication.

  • Ability to communicate with colleagues and business contacts in a professional, courteous manner as well as to work independently and also in a team environment.

  • Bilingual in English and Chinese.

provide community-based, multi-social services for older adults and immigrant families living in
Southern Brooklyn. Our mission is to help the Asian American immigrant community and other
disadvantaged persons to thrive through advocacy, civic engagement, and collaborating in partnerships.
