Multi-Social Services:
HCS case counselors provide free year-round benefit screening, enrollment assistance, and counseling support to anyone who needs it.
Through the HIICAP program (Health Insurance Information, Counseling and Assistance Program), we help Medicare beneficiaries make the most of their Medicare benefits and navigate through the myriad of complex plans that are out in the healthcare marketplace. We help clients make educated and informed decisions. This includes providing clients with information on Medicare Part D prescription benefits, Medicare Advantage Plans, Medicare supplemental insurance, long-term care insurance, Low Income Subsidy and the Medicare Savings Program.
In addition, counselors also assist low-income New Yorkers with free screening, counseling on Medicaid enrollment, elderly prescription insurance coverage (EPIC), Senior Citizen Rent Increase Exemption (SCRIE), Supplemental Nutrition Assistance Program (SNAP), Low Income Housing Energy Assistance Program (LIHEAP), Affordable Housing, Senior housing, NYCHA/Subsidized housing, and much more to prevent homelessness and assist families and individuals in need to maintain stability in their communities.
If there are any problem/need/issue, case counselors also provide supportive contact to help individuals address their problems of daily living, such as managing mail, making and remembering appointments, organizing daily activities and coping with specific personal or family problems.
